1. 일반

  ① 본인 및 부모의 신분증 또는 여권 사본

  ② 본인과 부모의 가족 관계를 증명하는 정부 발행 증명서 (Ex. 가족관계증명서, 출생증명서 등)

2. 한부모 가정인 경우

  ① 본인 및 부모의 신분증 또는 여권 사본

  ② 본인과 부모의 가족 관계를 증명하는 정부 발행 증명서 (Ex. 가족관계증명서, 출생증명서 등)

  ③ 정부 발행 한부모 증명서

3. 이혼한 경우

  3-1 부모님이 고등학교 입학 전 이혼 후, 재혼하지 않은 경우

    ① 본인 및 친권을 가지고 있는 어머니 또는 아버지의 신분증 또는 여권 사본

    ② 본인과 부모의 가족 관계를 증명하는 정부 발행 증명서 (Ex. 가족관계증명서, 출생증명서 등)

    ③ 이혼 증명서 (친권 관계 명시 자료)

  3-2 부모님이 고등학교 입학 이후 이혼한 경우

    ① 본인 및 친생 부모의 신분증 또는 여권 사본

    ② 본인과 친생 부모의 가족 관계를 증명하는 정부 발행 증명서 (Ex. 가족관계증명서, 출생증명서 등)

    ③ 이혼 증명서(선택)

  3-3 이혼 후, 친권이 있는 어머니 또는 아버지가 한국인과 재혼한 경우

    ① 본인 및 부모의 신분증 또는 여권 사본

    ② 본인과 부모의 가족 관계를 증명하는 정부 발행 증명서 (Ex. 가족관계증명서, 출생증명서 등)

    ③ 이혼 증명서

4. 부모님이 사망한 경우

1. 전문대, 직업대학교

위의 서류를 제출하지 못하는 경우, 첨부되어있는 사유서를 다운 받아 제출 불가한 사유를 작성 후 친필 서명하여 제출 하시기 바랍니다.

경우에 따라 사유서는 인정되지 않을 수 있으며, 관련하여 보완서류 제출이 요구될 수 있습니다.
